PAYMENT FORM

Please fill out this form with your details and upload an image containing your proof of payment. You can check the instructions for payment below. If you are paying on behalf of an organization, agency, or an office in cases of group therapy sessions for a family or workplace, please put the name of your organization/office/agency on the blanks for the first and last names. Once your payment has been confirmed, you will receive an email and text with the details of your confirmed appointment.  

 

NOTE: You have 24 HOURS to make a payment from the time you have sent your appointment request. If you fail to pay after the designated time, your slot will be given to our next client in the waiting list and you will have to set a new appointment.

Instructions for Payment

To make a payment, you can use the following methods:

 

VIA GCASH:

  • Open your GCash App using your mobile phone

  • Tap on "Pay QR"

  • Scan the QR code HERE and enter the amount you need to pay, then tap "NEXT", then "PAY"

  • Take a screenshot of the payment confirmation and upload.

VIA MAYA (Formerly PayMaya)

  • Open your Maya App using your mobile phone

  • Tap on "Pay with QR" (the Icon at the bottom middle part of yoru screen)

  • Scan the QR code HERE and enter the amount you need to pay, then tap "NEXT", then "PAY"

  • Take a screenshot of the payment confirmation and upload.

VIA BDO:

Method 1 (Bank Deposit):

  • Make a deposit to the bank account details below by visiting your nearest BDO branch 

  • Take a photo of the deposit slip/screenshot of the transfer confirmation and upload it at the bottom of this form.

Method 2 (Mobile Banking):

  • Login to your BDO Mobile Banking App

  • Tap "Send Money"

  • Tap "Via QR"

  • Tap "Scan QR" (You may need to allow the app to use your phone's camera)

  • Scan the QR code HERE. 

  • Enter the amount and confirm send.

  • Take a screenshot of the transfer confirmation and upload.

VIA LANDBANK:

  • Make a deposit to the bank account details below by visiting your nearest Landbank branch 

  • Take a photo of the deposit slip/screenshot of the transfer confirmation and upload it at the bottom of this form.
